How to choose a new vascular or vein specialist

  • Ask your primary care physician (PCP) for referrals — they know your medical history and local specialists.
  • Look for board-certified vascular surgeons or physicians with fellowship training in vascular surgery or venous disease.
  • Check whether the provider has experience in the specific procedures or therapies you require (e.g., sclerotherapy, endovenous ablation, venous stenting).
  • Review patient testimonials and practice information on provider websites and reputable medical society directories.
  • Confirm insurance coverage and whether the new provider accepts your plan before scheduling.
  • Consider proximity, office accessibility, and availability for follow-up visits.

Medical Records Access

Continuity of care depends on timely access to your medical records. Please follow these steps carefully.

Important dates
  • Practice closes (last clinical day): January 9, 2026.
  • You may request copies of records directly from our office until: January 30, 2026. (Requests made after this date must follow the procedure below.)
  • Long-term storage & retrieval: After January 30, 2026, all medical records are transferred to and securely maintained for 7 years (per Massachusetts law) by Morgan Records Management, LLC (MRM).
How to request your records before January 30, 2026
  1. Submit a completed and signed medical records release form to the practice. We will process requests for copies only with a properly completed release.
  2. You may request paper copies or an electronic copy where available. Fees for copying and postage may apply in accordance with state law.
  3. If someone else will pick up records on your behalf, they must present your signed release and valid photo ID.
How to obtain records after January 30, 2026

After transfer to Morgan Records Management, request records directly from MRM using their online system or contact options below:

When requesting from MRM, be prepared to provide:
  • Your full name (as on record), date of birth, and address.
  • Approximate dates of service and the name of the practice (John T. Frasca, MD / Specialty Vein Care Program).
  • A signed release form if required by their process.
HIPAA and security
  • All requests will be handled according to HIPAA regulations to protect your privacy. A signed authorization is required to release health information to anyone other than the patient.
  • For electronic delivery, please confirm a secure email address or request delivery through a secure portal. Avoid sending PHI to unsecured public email addresses if you have concerns.

What to Expect & Important Notes

  • No clinical care will be provided after January 9, 2026. Please plan follow-up care with your PCP or a new specialist.
  • Records may contain imaging reports, procedure notes, and operative reports required for continuity of care — request them early if you anticipate upcoming procedures.
  • Paper or electronic record request processing times vary; submit requests as soon as possible to avoid delays.
  • All records will be stored by Morgan Records Management for 7 years as required by Massachusetts law.
